Request for Citizenship Documentation

Per Federal Guidelines, a student is required to present their original citizenship document to the Office of Student Financial Aid Services before financial aid may be paid to the student's account.

In order to expedite the processing of your Financial Aid, please forward a copy of your Citizenship Documentation to our office. We will review your file using the copied document to award an unofficial financial aid package. You may fax(860)486-6629 or email the copied document to our office. Your financial aid will not be disbursed to your student account until you present the original document. Please stop by the Office of Student Financial Aid Services with the original documentation at your earliest convenience. If you are attending a regional campus you can present the original to their Office of Student Services.

Acceptable Documentation:

• Copy of student’s birth certificate showing that they were born in the U.S.
• Copy of U.S. passport book, current or expired (except limited passports)
• Copy of U.S. passport card
• Certificate of Citizenship, must have student’s name, certificate number, and the date the certificate was issued.
• Certificate of Naturalization, must have student’s name, certificate number, Alien Registration Number, name of the court (and date) where naturalization occurred
• Certification of Birth Abroad, form GS-545, DS-1350, or FS-240, “ Reported of Birth Abroad must have an embossed seal “United States of America” and “ State Department”
• Permanent resident Card or Resident Alien Card (Form I-551)
• Passport, must be stamped “Processed for I-551 with expiration date
• I-94, must be stamped “Processed for I-551
• With expiration date, or “Temporary Form I-551, “ with appropriate information completed
• Arrival-Departure Record, Form I-94, must be stamped as a Refugee, Asylum Status, Conditional Entrant (before 4/1/80), Parolee, or Cuban-Haitian Entrant
